Now you can apply for services online! Please follow the instructions below to ensure that all documentation is collected prior to applying online and agree to consent to the statements below. A submitted application does not guarantee that services will be rendered. All applications will be screened and will be determined by eligibility criteria. You will be notified within 3-5 days about our determination.
UPLOADED DOCUMENTS REQUIRED
In order to complete your application and receive services, the following scanned documentation is required. Documents can be scanned using a desk scanner, or by taking clear pictures of the documents with a phone or tablet. Documents will need to be uploaded on the onboarding application using the link below.
The following documents are needed to process your service request.
PLEASE INCLUDE COPIES OF THE FOLLOWING:
1. Government-Issued ID. Please upload one of the following:
• Driver License/ State ID
• Passport/Resident Card/Work Permit Card
• Consular ID Card/Foreign Country Photo Issued ID
2. Proof of Address – If your ID does not include an address, please provide documentation that verifies your current physical address. (Recent utility bill, phone bill, is acceptable).
3. Income Verification – Proof must be within last thirty (30) days for all household members 18 years and older, such as:
• Checks stub(s)
• Photocopy of CalFresh Card or letter from SSA
• Photocopy of Medi-Cal Card or letter from SSA
• Award Letter(s) (Social Security, SSD, VA, TANF, SSI, etc.)
• Pension Statement
• Child Support Statement
• Unemployment Income Statement
• Declaration of No Income Statement; if there is no income in the household or you are unable to get the documentation, please write or type a letter indicating that you or your household does not receive any form of income. Please upload this documentation with your application
COMPLETING THE APPLICATION:
Head of Household- When completing the online application, please be sure to use the female head of household/ female legal guardian as the main contact when completing the application; however, please indicate additional family members and the services being requested. If there is no Female in the applicant’s household, please use male head of household or male legal guardian. Unaccompanied youth are considered their own head of household
Social Security Number- Including your social security number in the application is optional.
Contact Information- In order to better serve you, please include a reliable phone number and/or email where Staff may be able to reach you. An unreliable phone number or email will delay your application from being processed.
PHOTO RELEASE CONSENT
By applying for services, I authorize Community Action Partnership of Orange County and affiliated programs such as their Community Centers to photograph, record audio and/or video of me or any household members receiving services may use and permit other persons acting on their behalf to use the recorded information for purposes including, but not limited to: educational, promotional, public relations, advertising, charitable goals and for such purposes and in such manner as may be deemed appropriate. I understand and agree that the recorded information may reveal my identity.
CLIENT CONFIDENTIALITY POLICY AND LIMITATIONS
Our Community Centers maintain a policy of respecting a client’s right to privacy and confidentiality. In order to achieve this goal we keep all records in locked file cabinets and secure databases.We are only permitted to share certain information on a need-to-know basis with appropriate staff, consultants and other authorized professionals. (A complete list is available upon request) We use confidential information to assess your needs and your family’s needs.We will also use confidential information in order to work cooperatively on your behalf with other agencies with whom you allow us to share information. However, California law mandates that our staff share certain confidential information under the following emergency circumstances:
• If we have reason to believe that any child, elderly adult or dependent adult is being abused or neglected, we are required by law to report the suspected abuse or neglect to Child Protective Services or to APS.
• If we have reason to believe that you are a danger to yourself or others, we are required by law to notify the appropriate authorities in order to ensure everyone’s safety.
